Wolfpack Waiver

      In consideration of receiving permission from the ISU Combat Club to participate as a member of the Combat Club and to participate in any activity, event, tournament, contest or meeting that the ISU Combat Club participates in, sponsors, attends, or supervises, the Undersigned releases the ISU Combat Club, Illinois State University, Bloomington-Normal, the owner(s) of any premises where any ISU Combat Club activity, event, tournament, contest and/or meeting occurs, and/or any other member of the ISU Combat Club from any and all liability, claims, demands, action, and/or causes of action arising out of or related to any loss, damage, and/or injury, including death, that may be sustained by the Undersigned, and/or any property of the Undersigned, while participating in any activity, event, tournament, contest, and/or meeting that the ISU Combat Club participates in, sponsors, attends and/or supervises.
      The Undersigned being duly aware of the risks and hazards inherent upon participating in any activity, event, tournament, contest, and/or meeting of the ISU Combat Club and elects voluntarily to participate knows that participation requires physical contact by others to the person of the Undersigned and that such participation may become hazardous and dangerous during the time that the Undersigned is involved in/with the ISU Combat Club. The Undersigned voluntarily assumes all risks of loss, damage, and/or injury, including death, that may be sustained by the Undersigned, and/or any property of the Undersigned while participating in any activity, event, tournament, contest, and/or meeting that the ISU Combat Club participates in, sponsors, attends or supervises.

      This release shall be binding upon the distributees, heirs, next of kin, executors and administrators of the Undersigned.
      In signing the foregoing release, the Undersigned acknowledges and represents:

• (a) That he or she has read the above release, understands it, and signs voluntarily;
  
• (b) Knows there is an inherent risk of injury involved with participation in the ISU Combat Club;
• (c) The Undersigned will not hold any of the fore mentioned accountable for any loss, damage, and/or injury to the Undersigned and/or their property while participating in any activity, event, tournament, contest, and/or meeting that the ISU Combat Club participates in, sponsors, attends, and/or supervises;
  
• (d) That he or she is over 16 years of age and of sound mind;
